Five Simple Steps to Refer Your Patients to Advanced Prosthetics
- Send the prescription.
Submit a prescription to Advanced Prosthetics, including diagnosis codes and supporting clinical notes. - Patient schedules an evaluation.
Your patient contacts their nearest Advanced Prosthetics clinic to schedule a free evaluation.
Please note: For diabetic shoe evaluations, we must receive both the primary care physician’s notes and referring provider’s notes before scheduling. - We complete the evaluation and verify benefits.
Our clinical team evaluates your patient and confirms insurance coverage before proceeding. - Device fabrication and fitting.
A certified prosthetist or orthotist fabricates and/or fits the prescribed device to meet your patient’s individual needs. - Follow-up and outcome reporting.
Your patient is scheduled for follow-up care, and we provide outcome updates to keep you informed.
